Technically, no but it’s important to understand how testosterone therapy works. When you supplement with testosterone, your body’s own production decreases or shuts off. If therapy is stopped, it can take time for natural production to recover, and many men feel worse than they did before starting during that transition period.

If testosterone therapy is recommended and started, it’s because your body is not producing what it needs. For that reason, stopping treatment without a clear plan is often not beneficial. Ongoing therapy and monitoring are typically the most effective way to maintain symptom improvement and overall well-being.

Testosterone therapy does suppress sperm production while you are on treatment. If fertility preservation is important to you, this should be discussed before starting therapy so appropriate options can be considered.

If you’ve been told your testosterone levels are normal but you still feel like crap, I want to see those labs. Symptoms don’t always line up with standard reference ranges. We look at total testosterone, free testosterone, SHBG, and how you actually feel. Treatment decisions aren’t based on a single number; they’re based on the full clinical picture, including symptoms and overall health.

Generally, no, routine blood donation is not necessary while on testosterone unless you have an underlying condition that causes excess iron storage, such as hemochromatosis.

Testosterone stimulates erythropoietin, which increases red blood cell production and can raise hematocrit levels. Donating blood may temporarily lower hematocrit, but it does not address the underlying cause and can create new problems, including iron deficiency anemia.

When hematocrit rises, we look for and address contributing factors such as sleep apnea, dehydration, dosing strategy, and injection frequency. Management should be individualized and medically guided.

Possible side effects can include acne, increased red blood cells, fluid retention, or mood changes. These risks are monitored closely with routine follow-up and lab work. If side effects occur, adjustments can often be made to improve tolerance and safety.
